Intervention Venues for Youth (IVY) Case Manager

Job Summary:

The IVY Case Manager provides the service planning, coordination, treatment, and implementation of home and community-based services to individuals below the age of 22 who have a mental illness and/or developmental disability.  The incumbent will also provide pre-emptive crisis planning and treatment to allow the individual to maintain his or her presence in the community.

Education/Experience

Required:  Bachelor’s Degree in human services or related field.

Preferred:  Master’s Degree in human services or related field.

Experience:  Knowledge of autism and developmental disabilities preferred

License/Certifications

Required:  Valid drivers’ license with current auto registration, insurance, and reliable transportation
