The Bridge Care Coordinator provides intake and care coordination to youth ages 3-22 who have developmental disabilities. The Care Coordinator also oversees Flexible Family Funding and Family Managed Respite. The coordination of these programs may occur in the community or home setting while following Medicaid, State and Agency Guidelines.
