School Based Case Manager

Job Summary:

This position coordinates the work connected to School Based Clinical Programing. This includes direct work with schools, families, and includes effectively completing assigned programmatic administrative tasks, and coordinating with all other CYSF programing. Intake and document support; admission, scanning, connecting with parents for signatures, external providers for records. Case management for SBC clients with medication, respite, CSW referrals. Summer supports as needed. CSP and tam meeting support. Potential transportation and short term skills work.

Education/Experience:

Requires: Completed Bachelor's Degree in education or human services filed. Minimum of one year experience with:

-working with children who have emotional and/or behavioral disorders

-working with children and families in a supportive or therapeutics relationship;

-Vermont's system of care for children and youth, and working with agencies that serve this population, i.e., DCF, schools, court diversions, etc.;

-working with children in a school or structures setting

Job Type: Full-time

Salary: $16.50 - $18.00 per hour
